Waiver, Release, and Indemnity Agreement

I certify that I am in good health and physical condition to participate in pickleball activities at Chicago Pickleball Academy. I understand there are inherent risks, including but not limited to bodily injury, heat exhaustion, and actions of other participants. By enrolling, I voluntarily assume all risks and release, waive, and discharge Chicago Pickleball Academy, Selvaggi Ventures LLC, its officers, employees, contractors, and representatives from any liability, claims, or damages of any kind. I further agree to indemnify and hold harmless the Academy against any claims, including attorney’s fees, arising from my participation.

 

Payment, Cancellation, and Credit Policy

• All payments are final.

• Refunds will only be issued in cases of injury or illness that prevents play. Documentation may be required.

• The Academy may securely store credit card information for ease of processing but will only charge with explicit authorization. Cards may be removed from file upon request.

 

Rackets / Paddles

• Players are expected to bring their own equipment.

• The Academy may provide paddles for clinic use upon request and may sell equipment at a discount when available.

 

Food and Water

• No food or chewing gum is permitted on the courts.

• Players must bring their own water in a closed, distinguishable container.

 

Email / Text Updates

• By registering for any Chicago Pickleball Academy program, players agree to receive email and/or text updates regarding scheduling, cancellations, promotions, and other Academy-related information.

• Contact information will not be sold or shared outside of Selvaggi Ventures LLC and its affiliates.

 

Photo & Media Release

• By registering for Academy programs, participants grant permission for their likeness (photo, video, or other media) to be used in promotional materials, including but not limited to social media, digital, and print advertising.

 

Program Levels

• The Academy reserves the right to place players into appropriate training groups based on skill level and coach evaluation.

• Coaches may move players between levels to maximize training quality.

 

Membership Non-Refundability, Non-Transferability, and Cancellation
*All Memberships are a 3-month commitment and can be paid monthly, quarterly, semi-annually, and annually*

1. All memberships are non-refundable and non-transferable.

2. Memberships cannot be pro-rated.

3. Only the individual whose membership is paid in full may attend programs.

4. Memberships may be canceled at any time; benefits will remain available until the end of the paid cycle but will not renew unless extended.

 

Membership Benefits

1. Benefits are only valid for the months purchased.

2. Members may prepay for multiple months in advance.

3. Benefits are not retroactive and do not apply to past programs.

 

Reservation Attendance & Cancellation Policy

• Attendance is expected.

• No-shows or cancellations made within 24 hours of a scheduled session may result in loss of membership privileges at the Academy’s discretion.

• Substitute players are not allowed for free or reserved sessions.

 

Additional Terms

1. Conduct & Etiquette: All members are expected to maintain good sportsmanship and respect. The Academy reserves the right to revoke membership for inappropriate conduct.

2. Equipment & Attire: Players must wear proper court shoes and athletic attire.

3. Liability: The Academy is not responsible for personal injury, loss, or damage to personal property. Participation is at the player’s own risk.

 

By purchasing and participating in Chicago Pickleball Academy programs, players agree to these terms and conditions.
